                              Death Benefit Claim


                              If this benefit is provided in your Core Plan, please note that claims must be reported within 90 working days following the date of death and the following documents must be provided: 
                              A fully completed Death Benefit Claim Form.
                              A death certificate.
                              A medical report indicating the cause of death.
                              A written statement outlining the date, location and circumstances of the accident (not applicable in the event of death due to sickness).
                              Official documentation proving the insured member’s family status, and for the beneficiaries, proof of identity as well as proof of relationship to the insured member.
                              Beneficiaries are, unless otherwise specified by the insured: 
                              The insured member’s spouse, if not legally separated.
                              Failing the spouse, the insured member’s children (including step-children, adopted and foster children) then living (children born within 300 days of the date of the insured member's death shall be considered living) or by representation, in equal shares among them.
                              Failing children, the insured member’s father and mother, in equal shares between them, or to the survivor of them.
                              Failing them, the insured member’s heirs.

                              Please note that in the specific case of the death of the insured member and one or all of the beneficiaries in the same occurrence; the insured member shall be considered the last deceased.
